NASA astronauts Butch Wilmore and Suni Williams finally touched down on Earth after an unexpected 286-day stay aboard the International Space Station (ISS), and let’s just say, gravity wasn’t the only thing hitting hard.
The pair, who were meant to stay in orbit for just a few weeks, have returned looking gaunt, fragile, and understandably a bit worse for wear. But their frail appearance is only the surface-level side effect of what spaceflight actually does to the human body. As their stretchers rolled out, NASA’s medical teams were already on standby—and for good reason.

From Baby Feet to Chicken Legs
Without Earth’s gravity, your muscles and bones basically stop bothering. Astronauts face significant muscle atrophy and bone density loss, even with a daily two-hour workout routine. The result? Legs that barely function, feet that look like they belong to a newborn, and a return to Earth that involves more hobbling than heroic striding.
The Gaunt Look and Dramatic Weight Drop
Nine months of space rations and rigorous exercise regimes have taken a toll on Wilmore and Williams. Their visibly sunken features and significant weight loss reflect just how physically intense life in orbit can be—even if you’re not spacewalking every day.

The Brain Fog Is Real
Turns out, it’s not just your body that struggles—your brain does too. NASA research has found that microgravity, stress, and lack of sleep can lead to slower cognitive processing, impaired memory, and even riskier decision-making. While most of this seems to bounce back after re-entry, it’s still not ideal when you’re floating above Earth trying to do high-stakes problem-solving.
Seeing Stars (And Not in a Good Way)
Around 70% of astronauts experience Spaceflight Associated Neuro-Ocular Syndrome (SANS)—a fancy way of saying your eyeballs get weird in space. With fluid building up in the skull, optic nerves can swell and retinas fold, leading to blurry or fuzzy vision. Some astronauts recover fully. Others don’t.

Radiation & Cancer Risk
Then there’s the exposure to cosmic radiation. Without the Earth’s atmosphere as a buffer, astronauts soak up a whole lot more radiation, putting them at increased risk for developing cancer later in life.
So, while space tourism might sound sexy, the long-haul effects are anything but. As NASA continues planning longer missions, including trips to Mars, the health toll is proving to be one of the biggest challenges yet.
