Ozempic And Vision Loss May Be Linked, Says Research

Ozempic and its cousins Wegovy and Mounjaro have become the buzziest names in medicine, pitched as weight-loss game-changers and hailed by TikTok wellness gurus as the “miracle jab.” These drugs, technically called GLP-1 agonists, work by mimicking a hormone that controls blood sugar and dampens hunger signals. But alongside their cult status, new research is shining a not-so-glam light on their possible impact on your eyesight.

Two studies published this week suggest that while the risks might be smaller than initially feared, people taking GLP-1 drugs may face a slightly increased chance of rare but serious eye conditions, including sudden vision loss, explains Flora Hui & Pete A Williams from The Conversation.

The main concern? Non-arteritic anterior ischaemic optic neuropathy, or NAION. It’s as terrifying as it sounds. Often dubbed an “eye stroke,” it strikes when blood flow to the optic nerve is blocked. “Unlike other eye conditions that develop gradually, NAION causes a sudden, painless loss of vision,” explain Flora Hui and Pete A Williams in The Conversation. In other words: you go to bed seeing fine, wake up and discover one eye isn’t working.

A 2024 study previously found people prescribed semaglutide for diabetes were four times more likely to develop NAION, while those taking it for weight loss faced almost eight times the risk. That prompted the European Medicines Agency to classify NAION as a “very rare” side effect, updating product labels to warn of a one in 10,000 chance.

The latest studies, however, suggest the picture may not be quite so dramatic. Looking at medical records of up to 185,000 people with type 2 diabetes in the US, one study found only 35 patients (0.04%) on GLP-1 drugs developed NAION, compared with 19 (0.02%) in a comparison group. Another study didn’t find an increased risk at all. Instead, it flagged a small uptick in diabetic retinopathy, a condition where high blood sugar damages tiny blood vessels in the retina. Interestingly, participants on GLP-1 drugs actually experienced fewer severe complications overall than those on other diabetes medications.

Still, there are important caveats. People with existing risk factors such as diabetes, high blood pressure or sleep apnoea should weigh up their choices carefully with their doctor. Optometrists can also check for “crowded” optic nerve heads – another NAION risk marker.

For now, the advice is simple: keep up regular eye exams, tell your eye doctor if you’re on GLP-1 medication, and seek immediate help if you experience sudden vision changes. As Hui and Williams stress, “Doctors should discuss NAION risks during prescribing decisions and work with eye care providers to monitor regularly for diabetic eye disease.”
